Elizabeth – Staying Connected in Rural Lancashire

For Elizabeth*, an older woman living with dementia who resides in a quiet village in rural Lancashire, being part of the local community is not simply a pleasant addition to life. It is central to her identity and wellbeing.


Before receiving the right care and support, Elizabeth spent a difficult period in a residential care home. The routine there did not suit her: strict schedules, unfamiliar surroundings, and limited personal attention left her feeling isolated and unsettled. The loss of daily freedom, particularly around mealtimes and outings, affected both her mood and her behaviour. It quickly became clear to Elizabeth and her family that a different approach was needed, one that would enable her to remain in her own home and continue participating in the life of her village.


In addition to ensuring her care needs were met, a key aspect of achieving this was transport. Living in a rural setting, Elizabeth was at risk of becoming cut off from local events, friendships, and community activities that played a vital role in her emotional and mental health. For her, going outside was not merely for occasional leisure activities; it was essential to her sense of purpose and belonging. It was evident that any care arrangement would need to include a carer who could drive and actively support her in maintaining these connections.


When Promedica24 became involved, the team placed particular emphasis on understanding Elizabeth’s lifestyle and preferences. Katrina, the local Care Manager, worked closely with Elizabeth’s representative to ensure that her needs were thoroughly assessed. This included recognising the essential role of mobility in her care plan. Carers were carefully selected not only for their experience but also for their willingness and ability to drive, making it possible for Elizabeth to attend yoga classes, community talks, and local social gatherings as she always had.


Through a care package funded by Lancashire County Council, Elizabeth now benefits from the support of two live-in carers who assist with personal care, mobility, meal preparation, hydration, and other essential aspects of daily life. However, what makes her care arrangement distinctive is the way social engagement has been made a central part of her routine. Regular outings and structured activities continue to keep her mentally stimulated and emotionally fulfilled.


There have been many small but meaningful moments along the way. On one occasion, a new carer still unfamiliar with the local area struggled to navigate the country lanes on the way to a railway-themed community event. Without hesitation, Gary Derbyshire, the local Care Consultant, stepped in to assist, ensuring that Elizabeth did not miss out on the event she had been looking forward to. It was a simple gesture but one that reflected a genuine understanding of the importance these connections hold for her.


Since beginning her care with Promedica24, Elizabeth has shown a marked improvement in her emotional wellbeing. While her physical health remains stable, she appears noticeably calmer, more engaged, and content. Her priorities for the future remain clear: to continue living in her own home, to stay connected with her community, and to be supported by people who understand what truly matters to her.


Thanks to her dedicated carers and a care plan built around her individual needs, Elizabeth is well placed to achieve this goal.
